Privacy Policy

Cam and Madi’s Promise, Incorporated (C.A.M.P) will not collect any Personal Identifiable Information, or  Protected Health Information about you unless you choose to provide it to us. Please be aware that entrusting us with your information will allow our disability advocates to serve you better. While providing the requested information is completely voluntary, failure to provide adequate details may delay or prevent your ability to obtain health coverage through the Katie Beckett and NOW/COMP waiver programs. Your information will only be used to perform to following duties:

· Defining the best options for coverage among the list of available state and federal options.

· Facilitation of completing and submitting the eligibility application.

· Providing direct assistance, and referrals to any applicable resource due to crisis intervention, provider needs, evaluations, waiver application appeals, complaints, or questions in regards to determinations of coverage eligibility.

C.A.M.P advocates and volunteers do not provide any medical services, nor do we give any medical advice.  We do work with several healthcare professional community partners who often serve our families' and their children.  You can find them on our Partners' page, or any of the organizations that participate in our resource fairs.  You may contact them anytime to seek qualified healthcare advice and services.  If you need a more specialized provider, we can work with you to make the appropriate referral, too.  For services that go beyond C.A.M.P's scope of practice, the organization nor its representatives, accept remuneration for referrals it makes to clients seeking other services or providers.  Our staff advocates do; however, review clients' health records and will adhere to HIPPA guidelines, and state regulations pertaining to the handling, security and confidentiality of medical, as well as,  educational records.  Again, any information provided to us will be securely maintained, and the information will only be used to achieve the specified objective for which your information was collected.  We seek to practice excellent data stewardship because your trust is so very important to us!  If you have any questions about this policy or believe your privacy rights with respect to your personal, or health information have been violated, please contact our Privacy Officer in writing at:
